受診者のみなさんに安心と信頼を提供する新宿つるかめクリニック

受診される方へのメッセージ

当院、新宿つるかめクリニックの西元慶治理事長は医師の米国臨床留学を強力にサポートをする「Nプログラム」を主宰として運営しております。その10期生としてニューヨークのコロンビア大学関連病院St. Luke’s-Roosevelt Hospital Centerで内科留学し、内科研修３年間のうち２年間ベスト研修医として表彰されました。さらに、ニューヨーク州立大学呼吸器内科フェロー研修、ピッツバーグ大学病院集中治療科フェロー研修を経て2006年帰国しました。

日本で数少ない、米国専門医資格を更新している米国内科専門医（日本で78名）、米国呼吸器内科専門医（日本で5名）、米国集中治療専門医（日本で7名）です（1）。米国専門医ですが、通常の日本の保険診療の診療報酬で診療をしております。若手医師の教育者としても日本全国で教えております(2)。また、国民のための名医ランキング2021～2023年版および2024～2026年版にも掲載して頂いております（3）。

外来診療というリソースが限られた中での診療ですが、診断が難しい病気の患者様の診断・治療にも携わってまいりました。例えば、成人では日本で16例目のPFAPAの患者さんを診断・治療し、月１回の発熱発作を無くすことができました。その症例報告はwebで閲覧できるので、患者さんが集まってきております（4, 5）。新宿つるかめではAI問診も開始となり、患者さんの話を聞くことに集中できる体制も整います。

かかりつけの患者さんには、科学的根拠（エビデンス）に基づく予防医療も含め、通院して頂ければ安心できる包括的な外来診療を提供しております。エビデンスに基づく予防医療を日本全国に広める米国内科学会予防医学推進タスクフォース委員会の委員長でもあります（6）

Dr. Keiji Nishimoto, Chairman of Shinjuku Tsurukame Clinic, run the N Program, which provides strong support for physicians pursuing clinical training in the United States. As a graduate of the program's 10th generation, Dr. Yaegashi completed an internal medicine fellowship at St. Luke's-Roosevelt Hospital Center, affiliated with Columbia University in New York. During his three-year internal medicine residency, he received the best resident award for two consecutive years. He further completed fellowships in Pulmonary Medicine at the State University of New York and in Critical Care Medicine at the University of Pittsburgh Medical Center before returning to Japan in 2006.

He is one of the few physicians in Japan who maintains active U.S. board certifications: Board-Certified in Internal Medicine (78 doctors in Japan), Board-Certified in Pulmonary Medicine (5 doctors in Japan), and Board-Certified in Critical Care Medicine (7 doctors in Japan) (1). Although he hold US board certifications, he practices under Japan's standard insurance-based fee schedule. He also educate young physicians nationwide (2). Furthermore, he have been listed in the “Top Doctors for the People” rankings for the 2021-2023 and 2024-2026 editions in Japan (3).

Despite the resource constraints of outpatient care, he has been seeing many patients with challenging conditions. For example, he diagnosed and treated Japan's 16th adult PFAPA patient, eliminating their monthly fever attacks. This case report is available online, attracting patients seeking care (4, 5). Shinjuku Tsurukame has also introduced AI-based medical chart support system, enabling us to focus more intently on listening to patients' concerns.

For our regular patients, we provide comprehensive outpatient care that includes evidence-based preventive medicine, providing reassurance and best chance of being healthy and longevity through regular visits. He also serve as Chair of the American College of Physicians Task Force on Preventive Medicine, working to spread evidence-based preventive medicine nationwide in Japan. (6)

This significantly increases the likelihood of diagnosis and treatment without repeating unnecessary tests.
